Integrated Health Home

Capstone’s Integrated Health Program provides a team of professionals working together to provide whole-person, patient-centered, coordinated medical, behavioral and social support services for Medicaid eligible adults with mental illness.

The Integrated Health Program is a free service for those who qualify. The program is designed to improve access to care; improve the overall health care experience; and improve health outcomes of participants.

The Integrated Health Program coordinates services for individuals receiving Habilitation Services who are not on another Waiver.

Nicole McCoy, IHH Director

Nicole has been with Capstone since 2016, working in the Integrated Health home (IHH) program as Care Coordinator, Intake Coordinator and currently as the IHH Director.  Nicole has a bachelor’s degree in Human services from Buena Vista University.  She has worked in the Mental Health field for 10 years.

Brooke Williams, Supervisor/Care Coordinator

Brooke has been with Capstone since 2015 working in the Integrated Health Homes program. Brooke is a care coordinator for Marion County as well as current IHH Supervisor. Brooke graduated from Buena Vista University with a double major in Human Services and Psychology.

 

 

Kadi DeGeest, Care Coordinator

Kadi started at Capstone in February 2018 and graduated from Iowa State University in 2017 with a BS degree in Psychology. 

Dalton Cooper, Care Coordinator

Dalton graduated from Buena Vista University in 2015 with dual degrees in Psychology and Human Services.  Dalton has been with Capstone since April of 2016, working in the BHIS program and is currently with IHH as a Care Coordinator.


Amanda Miller, Care Coordinator

Amanda graduated with a Bachelor of Science from Kansas State University. She has worked in the mental health field since 2013. She has been a Care Coordinator with Capstone since 2017.

Delaney Abarr, Care Coordinator

Delaney, preferred to be called Laney, is a dog mom of 4. She is born and raised here in Newton, IA where she still currently reside. Laney has been involved with mental health and human services since the age of 18 (9 years). She has a bachelor’s degree in human services and is currently working towards a master’s degree in mental health counseling. Her hobbies include getting tattooed and embarrassing herself by huffing and puffing on the treadmill.


Sarah Urias, Intake Coordinator

Sarah is the Intake coordinator for Jasper, Poweshiek and Marion counties.  She graduated from Buena Vista University with a Bachelor’s degree in Psychology and minor in Business. She has worked in the Mental Health field for 22 years.


Sherry Lambertus, Peer Support Specialist

Sherry has been with Capstone IHH since June 2018. She is the Peer Support Specialist for Jasper, Marion, and Poweshiek counties .

 

 


 

Chelsea Smith, IHH Nurse

Chelsea grew up with an autistic cousin that she says taught her the importance of acceptance and compassion for others at a young age. “I learned things from Shawn that the other kids or teachers in school couldn’t teach me.”  She volunteered in the Special Education classrooms in high school and provided respite services and in home health care for Shawn during this time. “We would get ready for school and go together”, Chelsea says. Chelsea continued her work with the disabled with Progress Industries for 5 years, starting at the age of 16. She pursued her nursing degree in 2012 and graduated from DMACC in Ankeny in 2016. Chelsea is starting her nursing career with us here at Capstone. She enjoys spending time with her dog, Bella and traveling the country, and the world.
