Community Support

Capstone has a 6 bed group home and 15 unit apartment building that offers Supported Community Living Services. Both of these units are HUD subsidized housing units. Capstone Supported Community Living offers access to staff 24 hours a day, but these staff are not on site continuously. 


Capstone Behavioral Healthcare Community Support Program offers assistance and training to adults with chronic mental illness who are living in their own apartments or homes in the community.  Services provided are goal oriented and tailored to meet individual needs. 

Services and Support include:

·          Crisis intervention

·          Individual and group skill building

·          Money Management

·          Personal and Household management skills

·         Accessing Community Resources

Admission Criteria:

Individuals appropriate for referral must:

·          Be at least 18 years of age.

·          Have a diagnosis of mental illness Axis I and/or Axis II.

·          Be eligible for Habilitation Services, State or County funding to cover service costs.

·          Demonstrate a willingness to participate in the program.

Trina Mullikin-Supported Community Living Director 

Trina has worked in a variety of health related fields for physical and mental disabilities for nearly 30 years. She started with Capstone in 2005 in the Residential Department and became Director of Supported Community Living in 2013. She is dedicated to making the lives of our supported individuals positive and Stigma Free.
